Mortgage Checklist

After you have received your Loan Estimate and notified us of your intent to proceed with your application the bank may request of you the following documents.

  • Copies of the last two (2) years W-2's
  • Copies of 1099's or Award Letters for social security, pension or retirement income
  • Copies of the most recent Paystubs covering one (1) month's income-showing year to date
  • Copies of the last two (2) years Federal Tax Returns (all pages & schedules) (You will need two (2) years federal business returns if you have more than 25% ownership in a Corp or LLC)
  • Copies of the last two (2) months of Bank Statements (full statements, not history printouts from online banking) for checking, savings, CDs etc-all pages for all accounts other than Westfield Bank
  • Copies of 401K, IRA & Investment Accounts- current statement- all pages
  • Name, Address, Phone and Fax Number of the Attorney you would like to close your loan.

For Refinance (additional documents)

  • Copy of your current Mortgage Statements
  • Copy of your Homeowner Insurance Declaration Page, showing coverage and annual premium
  • Copy of current real estate tax bill for all properties currently owned (primary, investment, second homes)- all parcels

For Purchase (additional documents)

  • Copy of executed Purchase & Sale Agreement of property being purchased, include all addendums
  • Copy of cancelled Deposit Check
  • Real Estate Agent's Name, Address & Phone number
  • If gift funds are allowed, please provide Donor Source, amount & verification of transfer of funds
  • If you are selling an existing home- please provide a Listing Agreement, executed Purchase & Sale contract or closing disclosure, if property is already sold

If Legally Divorced or Separated (additional documents)

  • Divorce decree- all pages (if receiving alimony/child support and you would like to consider using this source of income to qualify for your mortgage loan, please provide a copy of cancelled checks / direct deposit, etc. as verification for the most recent three (3) months.
